Put your clinical knowledge to work

What will you practice as a nurse reviewer?

Use your familiarity with clinical language and patient records to build a careful, organized review process.

Find the evidence

Practice reviewing provider notes, nursing documentation, results, and other chart sources to locate the information needed for an assigned review.

  • Identify the document, service date, and relevant finding.
  • Recognize when a chart needs additional review.
  • Keep your findings connected to their source.

Build review accuracy

Develop the habit of checking what the record documents against the instructions for the review you are completing.

  • Use a consistent abstraction process.
  • Identify missing or conflicting information.
  • Document questions clearly for follow-up.

A practical learning pathway

Build your chart review routine.

Practice moving from the review assignment to a clear, traceable finding.

  1. Understand the assignment

Identify the review question, applicable instructions, and information you need to locate.

  2. Organize the record

Confirm you are reviewing the intended record and locate the relevant documents and dates.

  3. Locate supporting information

Read the source documentation and identify the details relevant to the assigned question.

  4. Compare and abstract

Apply the review instructions and enter the supported information in the appropriate fields.

  5. Check and communicate

Verify your entries against the source and clearly describe any unresolved documentation questions.

Try the reviewer mindset

A sample chart review exercise

Fictional practice assignment: Locate the completed laboratory report referenced in a follow-up note and record its date, result, and source location.

The sample chart contains a test order and a note stating that the result was reviewed. Where would you look next, and what would you record if the laboratory report is missing?

See the practice approach

Check the laboratory section and any scanned reports for the referenced result. If you locate it, record the requested details and source. If you cannot locate it, document the missing information and follow the assignment’s process for clarification. Do not invent a result. This is a general documentation exercise, not a HEDIS® measure scoring rule.

Skills you can explain and demonstrate

Prepare to discuss your review process.

Work toward being able to explain where you looked, what you found, and how you reached your review finding.

  • Organized chart navigation

Describe how you locate relevant clinical documents without losing track of the review question.

  • Evidence-based abstraction

Show how each entry connects to the source documentation and the assigned review instructions.

  • Clear reviewer communication

Summarize your findings and identify the information that still needs clarification.

  • Professional preparation

Use case practice, self-assessments, and job readiness assistance to describe the review skills you are developing.

Explore a new direction for your nursing experience.
